1. FILL OUT REGISTRATION FORM COMPLETELY - (SEE LINK BELOW)

ROSTER/WAIVER FORM MUST BE FILLED OUT BY EACH PARTICIPANT PRIOR TO EACH SEASON

2. PAYMENT IS DUE AT TIME OF REGISTRATION. (NOTE: WE WILL HOLD CREDIT CARD PAYMENT UNTIL FIRST GAME)

3. WE ACCEPT CASH, CHECKS, VISA AND MASTERCARD FOR PAYMENT.
|
|
4. TEAM ROSTER/WAIVER FORM MUST BE COMPLETED BY ALL PLAYERS PARTICIPATING BY FIRST GAME.

|
5. A $50.00 CHARGE WILL OCCUR IF TEAM DROPS OUT AFTER SCHEDULING IS COMPLETE.

BY REGISTERING, YOU AGREE TO THE FOLLOWING PROCEDURES:
|
|
1. NO REFUNDS OR CREDIT WILL BE GIVEN FOR TEAMS FAILING TO SHOW UP FOR OR CANCELING SCHEDULED GAME TIMES. 2. GAMES CANCELED OR STOPPED FOR FIGHTING OR ANY OTHER CONDUCT DEEMED INAPPROPRIATE BY THE MANAGEMENT WILL NOT BE RESCHEDULED. 3. PROPER EQUIPMENT MUST BE USED IN GAMES INCLUDING BUT NOT LIMITED TO INDOOR/TURF OR FLAT SOLED ATHLETIC SHOES ONLY, TEAMS MUST DRESS IN MATCHING COLORED SHIRTS, HOME TEAMS ARE REQUIRED TO CHANGE IN CASE OF COLOR CONFLICT 4. ALL GAMES CONSIST OF TWO 23:00 MINUTE HALVES AND ARE REFEREED. FOR A LIST OF HOUSE RULES AND REGULATIONS PLEASE CONTACT MANAGEMENT.
